- Download and complete a budget proposal using the templated provided on the Queensland Mental Health Commission website at: https://www.qmhc.qld.gov.au/documents/budgetproposaltemplate4xlsx
- Familiarise yourself with the Mandatory Requirement information on the next page.
Complete all fields in the application form, then UPLOAD a completed copy of your budget proposal at Page 6, BEFORE you submit your final application form.
Any additional attachments such as letters of support can be uploaded at Page 8.

Changes to submitted applications and applications submitted past the grant closing date will not be accepted
